The SPIKES Model: Clinical Communication with a Survivor of FGM

When engaging with a girl or woman who has survived female genital mutilation (FGM), it is crucial to approach the conversation correctly from the outset. It's normal to feel uneasy about discussing FGM with a survivor. While the SPIKES model is commonly used for delivering difficult news, it can also be applied in this context.

SETTING UP THE ENVIRONMENT

Ensure that the discussion location is quiet and private, making the survivor feel secure. Confirm the presence of appropriate individuals and ensure the survivor is comfortable with them. Turn off devices to minimize disruptions and give the survivor your full attention.


PERCEPTION

Allow the survivor to express themselves and share their understanding of their experience. This allows you to grasp the survivor's perspective.


INVITATION

Ask the survivor if they are willing to discuss their FGM experience. This empowers the survivor to choose whether to engage in the conversation.


KNOWLEDGE

Offer information to the survivor in manageable, factual portions. Use simple language and be direct, yet compassionate.


EMPATHY

Validate the survivor's feelings and show understanding without assuming their emotions.


STRATEGY & SUMMARY

Summarize the discussion briefly to ensure the survivor comprehends the information shared. Consider providing additional resources and planning for follow-up discussions or referrals.
